Stuck on making a Y-pipe

Good evening everyone,

I have tried several times, researched countless posts, and tried a handful of different CAD software and still have not come up with a solution.

I am trying to make a bifurcated duct for a project to model for flow simulation. I have designed this and made the specs myself with some guidance.

I have attempted using splines in order to create sweeps but have failed when it comes to joining the piece as a whole. I have also tried Lofting but it

neither produced the look desired and still failed when joining the parts.

Some details: S-shape, begins rectangular and ends in a circular geometry, constant area throughout...

As stated, this is designed myself so I cannot put the dimesions up, just asking for some advice please. I have seen that there are solidworks users

that have pulled off the Y shape no problem. Please see the picture below. I am a student and I don't mean to sound like a punk but the pic is All
