I am having trouble with some recurring errors. Do you have any ideas?

Technical Environments: 3DEXP R2023x FD03 (On-Cloud)

Problem Description:

1. Selecting "Role Remove" causes an error message to be displayed.
2. Selecting "Remove User" causes an error message to be displayed.

Scenario: 

1. Selecting "Role Remove" causes an error message to be displayed.

1-1) Admin users login and open the "Members Control Center".
1-2) A user select and "Remve Role: CSV (Collaborative Industry Innovator)"

1-3) Role removal failed with an error message: "Unable to perform action. Please contact system administrator."

2. Selecting "Remove User" causes an error message to be displayed.

2-1) Admin users login and open the "Members Control Center".
2-2) A user select and "Settings".

2-3) Romve user failed with an error message: "Multiple relationships found for RO{UserRole,from IFW-c00009295364-R1132100850028,to R1132100504389}"

    
Actual Result : Remove Role & User failed to continue
