With reference to the second video in this post , in order to use the Approval Route Template through the Questionnaire for approving the Controlled Document , the logged in user needs to turn off the 'Change Required' setting on the CCD. 
However only an Administrator of the collaborative space has the permission to turn off this setting , so what is the proposed way of working here QUC-OCR2025xQuestionnaire
