My title bar is missing tabs; home, insert, annotate, etc.  As well as the menu dropdown and help button.

It is essentially like I do not have a "tilte bar"  

This is not a "title block" issue.  It is the "title bar" that is messed up.

Please help, this is getting extremely frustrating.
