Toolbox Incorrectly Sizes Fasteners

I recently switched to a new computer and switched from SW2011 to SW2012.  Since making the change, I no longer get correctly sized fasteners when I drag and drop them from the Toolbox. 

When I drag something from the toolbox, depending on what type of component it is (washer, bolt, etc), one of two things happens:

1) A message appears as I hover over the hole stating "There is no matching size for this component".  If I release the mouse button, the fastener that is inserted is the smallest of the type.

OR
2) No message appears as I hover over the hole, but when I release the mouse button, the fastener that is inserted is one size too small for the hole.  For example, if the hole is M8, a M6 bolt will be selected.

Response 1 seems to happen most often with washers.  Response 2 seems to happen most often with bolts.  The same behavior occurs regardless if I'm inserting a fastener into a hole wizard hole or a hole made by an extruded cut.  It also makes no difference which toolbox group I use (ANSI Inch, ANSI Metric, or MIL) nor does it matter if I work in mm or inches.
