I have this scenario in my workflow, when I proced with the first file approval the transition operation increment the revision to the value 00 and set atomatic a revisio description like " first approval". In the follow approval transition after the first I need to set a custom revision description, I remember that in a company where i worked in the past, when I proced with the second o third revision transition appeared on screen a box where digits the custom revision description, and if you proced with 7 files for sample, this box appeared 7 times.
