I have tried multiple time to create this mold form and it fails consistently.

I had one successful creation but, then I noticed that the face of the louver tilts back about 10 degrees. When I tried to create the form tool with the canted face I started running into problems. I have tried both with and without parting lines and I am still unable to create what I have pictured in my hand.
