how to turn on revision grid SW 2010?

I have received a sheet format from a customer which does not include revision grid.

How can I easily turn on revision grid in Solidworks 2010?

Additionaly I would be gald if new revision ballon shows up it would automaticaly fills up a revision table. Is it possible?

PS I call revision grid which might be called as well as drawing zones.
