Like the subject says, I have a file with the following history:

The problem here is that the revision is listed before the file enters the state (Approved) where the revision number is set.  A normal history looks like this:

This has happened twice (that I know of) in the last few weeks.  The only reason I know that this happened is that a user accidentally initiated a change on an approved drawing and model.  He asked me to roll it back to Revision 0.  I have done this many times in the past with no issues.  For the files with the messed up history, rolling back to Rev 0 leaves the files in the Approval Pending state which is incorrect.  The only way to 'fix' the problem is to roll it back to Transition from 'Approval Pending' to 'Approved' which leaves the file with a history that is 'different' from the norm.  I don't like unpredictable.  Software should not be unpredictable.
