Drawing revisions and dates when replacing old drawing

I have a library of old 2D drawings produced before my company moved to SolidWorks. I am now starting to replace these drawings with new versions (as and when a revision is required). These are probably stupid questions, but I wanted to check that what I have done in the past is correct.

What is the standard practice for:

  1. Starting revisions: i.e. if my old 2D drawing is on revision C, would I start my new SolidWorks drawing as D, or start over?
  2. Should the revision table be copied over from the old drawing?
  3. Is the date of the drawing the date I have drawn it, or the original date of the 3D drawing?
