I've been asked to see if we can customize the standard Solidworks BOM to match the photo attachment.  The key areas to customize are as follows

1.  Add BOM header/title called "Parts List" (not sure how to do this)

2.  BOM starts from the bottom up (not sure how to do this)

3.  Custom column labels (I know how to do this)

4.  The split cells in one column for "QTY Required".  Not sure if thats even possible.

Any feedback/direction would be greatly appreciated.
