Has anyone ever seen this before?! and if so, how the heck do you fix it?

P.S.

I have tried damn near everything with toggling Command Manager as well as hit the F11 key multiple times...

the only thing that worked was having a file (drawing or model) open and toggling the RMB Command Manger.

But now it has happened again and the previous stupid fix is doing nothing!

please help!
