Long story short, it seems add-ins are not creating the toolbar buttons anymore in 2011, per this api flag, EdmMenuFlags.EdmMenu_HasToolbarButton.  The context menu still shows up, commands work fine.  I've tested some of the other flags and they seem to work correctly, including writing a new simple add-in almost to the T of the example in the api refrence guide.

Anyone else?
