Hello Solidworks Community 

I am not very familiar with sheet metal so it might be a very simple thing but i could not figure out right way to design this V Band Clamp, so as per attached you might able to see what i want to achieve but i cannot make base flange from this sketch, probably it need a linear edges. but then how would i make top eyes to insert bolt 
But when i manage to make it somehow i will not be able to convert to sheet metal/ unfold it 

i need to unfold so i can cut slots on flat and then bend it. and show similarly on drafting drawings

Please teach me
