Please help me!

I need to wrap a 18 mm material about two surfaces which are not in the same plane(in the picture attached i draw the outside perimeter to which i want to wrap). I attached a part and the picture with some information. I've tried using swept boss/base but it's only wrapping about one curve, not both of them. Also I've tried to use lofted boss/base but it doesn't work also. 

Please provide some help.
